ACCESS FEE
-Our annual access fee is $525 per patient per year.
-Children of members between the ages of 17- 26 do not pay an access fee.
-The access fee does not cover medical services provided by health insurance plans. We will continue to submit claims through your insurance carrier in the traditional fashion.
-We are more than happy to work with you on payment plan options.
INSURANCE
-We will continue to submit claims through your insurance carrier in the traditional fashion.
-We will accept most major insurance carriers including Medicare and Medicare Advantage Plans.
-The office will require copay at time of service
-Please understand, it is your responsibility for any deductibles, coinsurance and other out-of-pocket expenses.
-You may be responsible for any bill or portion of a bill that is not paid by your insurance company.
-We will make every effort to provide services within the scope of your insurance policy.
If you are self pay or choose not to use your insurance for the visit we will discount the visit if paid in full at time of visit.
Florida Snowbirds: We will be licensed in Florida for telehealth so that we can perform visits for you if the need arises.
